Cancer is the leading cause of death for patients 35 to 64 years of age and the overall second leading cause of death in the United States. y More than 15 percent of patients with systemic cancer suffer neurological complications. An estimated 120,000 individuals are believed to experience consequent morbidity from either the direct or the remote effects of a systemic neoplasm on the neuraxis. One fifth of the yearly cost of cancer in the United States is relegated to therapy for patients with primary or secondary cancer of the nervous system. Reflective of the repetitive hospitalizations, neurological morbidity, specialized therapy, and extended rehabilitation required, this care commonly exceeds $100,000 per patient. y

Several autopsy and clinical reports suggest a trend of increasing incidences of intracranial and leptomeningeal metastases. Although this trend may in part be related to improved imaging techniques, patients with cancer who benefit from improved supportive care live longer and may experience the morbidity of wider dissemination of metastases. As the treatment of cancer becomes more vigorous, the nonmetastatic effects of cancer, including the complications of radiation therapy and chemotherapy, may be more apparent and more disabling. Recognition of these complications, requiring familiarity in both neurological disease as well as the common causes of neurological dysfunction in patients with cancer, places an increased burden for early recognition and intervention on the neurologist and internist.
