The introduction of insulin therapy for the management of diabetes mellitus is one of the greatest milestones in the history of medicine. Beta-cell replacement therapy is the only treatment that reestablishes and maintains long-term physiologic normoglycemia,81 because intensive subcutaneous insulin regimens cannot completely mimic the physiologic fluctuations of in vivo insulin secretion.82 The two options for replacement of beta-cell function in patients are whole-organ pancreas transplantation and islet cell transplantation. Although diabetes mellitus is not generally managed by the endocrine surgeon, whole-organ pancreas transplantation and islet cell transplantation are addressed briefly here for completeness. These topics have been expertly reviewed,81"83 and the reader is referred to these and the referenced studies for further detail.
