Epidemiology and Risk Factors

Histoplasmosis (Histoplasma capsulatum) is most common in basins of the Ohio and Mississippi rivers. Blastomycosis (Blastomyces dermatitidis) is found in this same area of the United States as well as in portions of Canada. Coccidioido-mycosis (Coccidioides immitis) is prevalent in desert areas of the southwestern United States, and paracoccidioidomycosis (Paracoccidioides brasiliensis) is the most common endemic mycosis in Central and South America. Airborne exposure to local soil is a risk factor. In South Asia and Southeast Asia (especially India, China, Thailand, and Vietnam), Penicilliosis marneffei is another significant endemic mycosis affecting HIV/AIDS patients (Randhawa, 2000).

Aspergillosis occurs most often in the context of immune suppression, especially in HIV-infected patients with CD4+ lymphocyte counts less than 50 cells/pL. Leukopenia, systemic corticosteroids, bone marrow transplantation, and broad-spectrum antibiotic therapy also increase risk for aspergillosis.
