Evacuation of Subungual Hematoma

Trauma to the nail causing bleeding between the nail plate and nail bed can be excruciatingly painful. Subungual hematomas can be drained easily in an office setting by creating a 2-mm hole in the nail and draining the blood, using a handheld cautery unit or heated end of a metal paper clip. If trauma was significant, with the hematoma involving more than 50% of the nail, the provider should consider a nail bed laceration requiring primary closure, and this should be radiographically evaluated. Frequently, phalanx tuft fractures can underlie the hematoma after significant trauma (Wang and Johnson, 2001).
