Incisions should be made perpendicular to the skin with symmetric edges and no angulation. Poor technique may jeopardize healing. For best results, the physician should make an ellipse with the length three to four times the width. If a portion of a pigmented lesion has a worrisome appearance, complete excision is recommended. The American Cancer Society uses the ABCDE mnemonic to recognize lesions of most concern: A, asymmetry; B, border irregularity; C, more than one color; D, diameter greater than 6 mm; and E, increased elevation or enlargement. If a lesion has one or more of these concerns, an excisional biopsy should be considered. In other lesions, the clinician may consider a shave biopsy or punch biopsy.
