Key Treatment

Treatment starts with high-dose corticosteroids in ITP patients with platelet count less than 20,000/^L and mucosal bleeding (Cines and McMillan, 2003) (SOR: B).

Splenectomy is indicated in ITP patients in whom steroids fail to sustain platelet counts greater than 30,000/^L (Nabel, 2009) (SOR: C). Patients should receive pneumococcal, H. influenzae, and meningococcal vaccines 1 to 2 weeks before splenectomy (Nabel, 2009) (SOR: C).

Patients with TTP are best treated with plasma exchange transfusions (Rock et al., 1991) (SOR: A). Patients with chronic thrombocytosis can be treated with a platelet-lowering agent such as aspirin or hydroxyurea (Harrison et al., 2005) (SOR: A).
