Key Treatment

trials are an option for patients with advanced-stage disease. Patients with advanced melanoma should be referred to a medical oncologist to consider adjuvant therapy with interferon-alpha. This may offer a small benefit in terms of recurrence-free and overall survival (Garbe and Eigentler, 2006).

For 2 to 3 years after any diagnosis of melanoma, patients should be seen every 3 to 6 months because most metastases occur in the first few years. If no new tumors are found, annual full-body skin examinations should be performed for life because of the higher risk of subsequent primary melanomas. Strict sun protection and sun avoidance should also be followed.

World Health Organization recommendations for excision margins are 5 mm for melanoma in situ lesions, 1 cm for lesions less than 1 mm, and 2 cm for melanomas 2 to 4 mm in thickness (Lens et al., 2002) (SOR: A).

Sentinel lymph node biopsies are recommended for melanomas 1 mm or greater in depth, although it is unclear whether this improves survival (Tanis et al., 2008) (SOR: B). Interferon-a may offer some benefit in recurrence-free and overall survival of melanoma patients (Garbe and Eigentler, 2006) (SOR: B).
