Potent topical steroid ointments are a good choice for first-line therapy of psoriasis (Afifi et al., 2005) (SOR: A). Clobetasol successfully treated 68% to 89% of patients with psoriasis (Nast et al., 2007) (SOR: A).

Topical calcipotriene and tazarotene show comparable efficacy as clobetasol for psoriasis (Afifi et al., 2005) (SOR: A). Topical steroids and calcipotriene or tazarotene show increased efficacy in psoriasis and fewer side effects (Afifi et al., 2005; Nast et al., 2007) (SOR: A).

Tacrolimus (0.1 % ointment bid) helps most patients with facial and inverse psoriasis (Brune et al., 2007; Lebwohl et al., 2004; Martin et al., 2006) (SOR: B).

Narrowband UVB is safer and more effective than broadband UVB for psoriasis (Ibbotson et al., 2004) (SOR: A) Methotrexate (5-15 mg/wk) is effective for widespread psoriasis not responding to topical treatments (Saporito and Menter, 2004) (SOR: A).

Acitretin (Soriatane) is used for unresponsive widespread or palmar-plantar psoriasis (Pearce et al., 2006) (SOR: A). Etanercept (Enbrel) is especially valuable in patients with psoriatic arthritis or cutaneous psoriasis (Nast et al., 2007) (SOR: A).
