Sickle Cell Trait and Sudden Death

Sickle cell trait occurs in 6% to 8% of the African American population (Kerle and Nishimura, 1996). Among black military recruits, the likelihood of sudden death is 28-fold higher in individuals with sickle cell trait than in those without (Kark et al., 1987). Most of these deaths are associated with exertional rhabdomyolysis. This typically occurs when the athlete is poorly hydrated, exercising in hot conditions, at altitude, or unconditioned. Athletes with sickle cell trait are generally not restricted from competition; however, consideration should be given to screening high-risk populations, with special precautions taken to ameliorate risk.
