Spectrum of Gender Identity and Expression

People may seek medical assistance in changing their physical sex to be congruent with their internal self-perception. Transsexuals usually desire full hormonal transition and sex reassignment surgery. Cross-dressers (previously referred to as

"transvestites") are persons who at times may dress as the other gender to be publicly perceived as such or for sexual pleasure. A transgendered individual is one who seeks to take on the social role of the other gender, either full- or parttime, often with the assistance of hormone therapy, but does not desire genital surgery. Intersex (transsexual) is a different medical concept and refers to persons born with ambiguous genitalia or for whom phenotypic and chromosomal sex do not match (e.g., 5a-reductase deficiency). Some persons do not perceive themselves as being fully male or fully female and seek to create new gender identities, similar to those found in many indigenous cultures. Reliable population prevalence estimates for transgendered and transsexual people are difficult to obtain and vary among cultures. In Belgium, overall prevalence was 1:12,900 for male-to-female (MTF) and 1:33,800 for female-to-male (FTM) transsexuals (DeCuypere et al., 2007). People who identify as transgendered or who cross-dress, however, are likely to be more numerous than individuals seeking sexual reassignment surgery.
