The human prostate is a major site of disease, especially in elderly men. An increasing number is affected by prostate cancer and orbenign prostate hyperplasia (BPH). Established risk factors are the presence of androgens and age (Cook and Watson 1968). In addition, genetic factors and environmental influences such as high saturated fat, low fruit vegetable diet and decreased sunlight exposure (vitamin D production) and life style also influence the risk for prostate disease (Stanford et al. 1999). In benign prostate hyperplasia, the presence of the androgen DHT that stimulates prostate growth through androgen receptor activation seems to be critical and reduction of DHT levels by inhibition of 5a-reductase is a successful therapy (Bartsch et al. 2000). In addition, alterations of the AR signaling, function and structure are associated with the progression of prostate cancer from a hormone-sensitive to a therapy-refractory state.

Effectiveness of Early Detection and Intervention

Medium- to high-intensity behavioral interventions produce consistent, sustained, and clinically important changes in dietary intake of saturated fat, total fat, fruits, vegetables, and fiber. The most effective interventions generally combine education, behaviorally oriented counseling, and patient reinforcement and follow-up (Ammerman et al., 2002). More intensive interventions and those of longer duration are associated with greater benefits and more sustained changes in diet. Research Evidence to Date

Very small amounts of fat are essential to meet basic nutritional needs, and fat is found in most foods, including vegetables and grains. Not the presence, but the type of fat makes the difference. Animal products produce saturated fat, which increases cholesterol levels, while the fat contained in most vegetables is unsaturated. Unsaturated fat reduces cholesterol. Other natural health products which are possibly effective include an aloe vera extract 0.5 hydrophilic cream applied topically three times daily, and an omega-3 fatty acid based lipid infusion (with both DHA and EPA) given intravenously. There seems to be a connection between the gut and spondyloarthropathies. Dietary Recommendations

The 2008 ADA guidelines considered the risk and advisable intake of carbohydrates, fats, proteins, and other food ingredients. Monitoring carbohydrate by counting, exchanges, or experience-based estimation remains a key strategy in achieving glycemic control. Epidemiology And Etiology

The majority of CAM treatments have been used to treat depression, including St John's wort (Hypericum perforatum), S-adenosyl-methinionine, and omega-3 fatty acids. In a Cochrane review of St John's wort for treatment of major depression, great heterogeneity was found among the 29 analyzed trials (Linde et al., 2005). St John's wort was found to be superior to placebo and similarly effective as standard antidepressants, but findings were more favorable to St John's wort studies from German-speaking countries, where use of the extract has a long tradition. The more positive results may be caused by physician expertise with the medication, patient selection, or flawed methodologies in some research. The larger, placebo-controlled studies have yielded mixed results, although compared to antidepressants, St John's wort has generally been better tolerated (Shelton, 2009). Nonpharmacologic Interventions

Therapeutic lifestyle change (TLC) is first-line therapy for patients at risk for cardiovascular events. Patients who smoke should stop. The amount of daily consumed cholesterol should be less than 200 mg. Table 27-2 summarizes the distribution of calories from nutrients. Reduced saturated fat and increased consumption of mono- and polyunsaturated fats promote serum LDL-C reduction. The ingestion of viscous fiber and plant stanols decrease cholesterol absorption. Ideally, patients should exercise for 20 to 30 minutes five times per week. Regular exercise promotes weight loss and relieves visceral adiposity and insulin resistance.

Purchase and storage

Many foodstuffs deteriorate when stored. In particular the labile, readily oxidized pigments, unsaturated fats and fat-soluble substances are destroyed. Fat-soluble vitamins are all reduced during storage, as are unsaturated fatty acids, and rancidity of fats reduces acceptance. The synthetic forms of vitamins A and E are more stable than their natural forms but have little antioxidant capacity. B vitamins are relatively resistant to breakdown during storage, but riboflavin will be lost when exposed to light. Compound feeds may have permitted antioxidants added to them to guard against various forms of deterioration but these will not stop all forms of deterioration for ever. Acceptability of all foods is reduced on deterioration.

Infants and Toddlers

The composition of human milk varies by time, day, and maternal nutrition and from woman to woman. Infant formulas contain about 50 more protein than human milk and, like breast milk, provide 40 to 50 of energy as fat (Table 23-4). Beginning at 2 years of age, fat calories should decrease to approximately 30 of total energy consumption, with less than 10 of calories from saturated fat, and dietary cholesterol less than 300 mg day (AAP, 2009).


It is recommended that all healthy children older than 2 years follow a diet in which a wide variety of foods provide adequate caloric intake to achieve proper growth and development as well as desirable weight. Total fat and saturated fat intake should be no more than 30 and 10 , respectively, of total calories dietary cholesterol should be less than 300 mg day. Children who are overweight or obese and have a high TG or low HDL level should be counseled regarding proper diet and increased physical activity. Children at especially high risk based on familial hyperlipidemias or premature cardiovascular disease may undertake a diet restricting saturated fat to 7 of total calories and cholesterol to 200 mg day (Daniels and Greer, 2008). Choice of food

Fats and proteins are absorbed much more easily than carbohydrates. The epithelial cells lining the gut of anorexic or starved patients can be very fragile so provision of an easily absorbable diet will reduce the risk of diarrhoea. Fat also yields twice the calories of carbohydrates and protein so a high-fat diet containing proteins of high biological value is ideal for most hospitalized patients. Proteins and fats further complement one another as non-protein calories are required to oxidize the amino acids during protein catabolism. Contraindications would include patients with EPI or other gastrointestinal problems, which would require a specialized diet low in fat, or renally compromised patients, which certainly would not benefit from a high-protein diet. Fats and Oils

People with diabetes are more sensitive to cholesterol in the diet, and it's recommended that less than 10 percent of the total caloric intake should come from saturated fat. If your LDL cholesterol is greater than 100 mg dl, than just 7 percent is recommended. It's also advisable to consume no more than 200 mg of dietary cholesterol each day (see Chapter 4). On the other hand, monounsaturated fats are terrific, as are the omega-3s. To reap the benefits of omega-3 fatty acids, it's recommended that you eat two to three servings of fatty fish each week. Or, try adding ground flaxseeds to your food.

The following list provides you with strategies to lessen the onset and intensity. Keep in mind, different things work for different people. And always check with your physician before starting any supplement (feverfew, omega-3s, calcium, or a multivitamin mineral).
