The most common form of vesicovaginal fistula in North America is injury to the bladder during hysterectomy.

In developing countries, the main cause is childbirth. Invasive cervical cancer or radiation therapy for this disease is also a contributor to the formation of vesicovaginal fistulas. Most fistulas resulting from hysterectomy occur during removal of the uterine cervix. The vaginal portion is usually located at the area where the vaginal cuff was sutured together, identified by the linear vaginal scar.
