The Benefits of Surgical Intervention in Advanced Cases

There is no doubt that the majority of patients presenting with advanced disease will die within 12 months if treated conservatively. The lack of an efficacious chemo-therapeutic agent which has been shown to consistently result in a response means that these patients have limited options available to them. In those patients with a reasonable performance status palliative surgery provides a means to reduce pain, remove unsightly fungating lymph node disease, and allow the patient to regain some aspects of their bladder and bowel function. More importantly, it gives the patients a more dignified period to at least go home and be near their relatives. As the disease already has a tremendous psychological burden on the patient, it is always a positive step to be able to walk around at home with better pain control and social support of the family.
