Negative blood cultures are reported in approximately 5% of confirmed IE cases, often delaying diagnosis and treatment.3,36, Sterile cultures may be the result of previous antibiotic use, subacute right-sided disease, slow growth of fastidious organisms, nonbacterial endocarditis (e.g., fungal or intracellular parasitic infections), noninfect-ive endocarditis, or improper collection of blood cultures. If nonbacterial or fastidious organisms are suspected, additional testing is essential. The choice of treatment regimen depends on patient history and risk factors.
