Pathophysiology

In ITP, IgG autoantibodies bind to platelet-antigen complex and are cleared from the blood via binding to mononuclear phagocytes by macrophage Fc y receptor, leading to thrombocytopenia. Platelet survival time is significantly shorter in patients with ITP (from minutes to 2-3 days). Sequestration in spleen, liver, and bone marrow is partially responsible for decreased platelet survival. Megakaryopoiesis may be reduced due to antibody binding to megakaryocyte precursors in the bone marrow.
