Patient Encounter Part 1

Jessica is an 18-month-old female who is brought to her pediatrician because of difficulty gaining weight. Her mother states that Jessica has had four to five loose stools daily "ever since I can remember." She was previously diagnosed with reflux and milk allergy. Oral ranitidine and elimination of cow's milk-based dairy products have not helped. Jessica is plotted on the growth chart at less than the third percentile for both height and weight. Mom also reports that she has been treated in the emergency de partment for pneumonia twice since birth. Mom is concerned because her mother's older sister died from some strange "wasting illness" when she was a young child.
