Identify your treatment goals for JJ in terms of maintenance immunosuppressants.

Create a plan for maintenance therapy for JJ, making sure to compare and contrast the pros and cons of the different maintenance immunosuppressants.

Identify the potential DDIs that may exist with the addition of the maintenance immunosuppressants and the medications that JJ is currently taking.

• Prophylaxis: antimicrobials given to prevent an infection;

• Empiric: preemptive therapy given based on clinical suspicion of an active infection; and

• Treatment: antimicrobials given to manage a documented infection.

Post-transplant infections generally occur in a standard pattern; therefore, prevention is a key management strategy. The information in this section is designed to only highlight prophylaxis options routinely used in organ transplant recipients. Please refer to Table 55-7 for a list of agents used for the prevention of Pneumocystis jiroveci pneumonia and CMV disease.
