Ventricular remodeling is a process that occurs in several cardiovascular conditions including heart failure and following an MI. It is characterized by left ventricular dilation and reduced pumping function of the left ventricle, leading to cardiac failure.7 Because heart failure represents one of the principal causes of mortality and morbidity7 following an MI, preventing ventricular remodeling is an important therapeutic goal.

Angiotensin-converting enzyme (ACE) inhibitors, angio-tensin receptor blockers (ARBs), P-blockers, and aldosterone antagonists are all agents that slow down or reverse ventricular remodeling through inhibition of the renin-angiotensin aldosterone system and/or through improvement in hemo-dynamics (decreasing preload or after-load). These agents also improve survival and will be discussed in more detail in subsequent sections of this chapter.
