This is normal anterior translation that can occur between C2 and C3 and less frequently between C3 and C4 in patients younger than 8 years. This displacement is secondary to the increased ligamentous laxity and transverse facet orientation seen in young children. The posterior cervical line (spinolaminar line ofSwischuk) is used to distinguish pathologic displacement from normal anterior displacement. A line is constructed connecting the anterior aspect of the spinous processes of C1 to C3. If the anterior aspect of the spinous process of C2 is more than 1.5 mm from this line, an injury should be suspected.
