Evidence of clinical and functional improvement is readily available, particularly as it relates to specific conditions. The use of a structured assessment integrated with a functional progression is very appropriate in most nonsurgical patients. A key to success in the more complex situations is the communication between the physician and therapist as they must be certain to provide a consistent message reinforced by each during their interactions with the patient. The functional progression approach is still used with these more challenging patients but with the recognition of special limitations and also expected outcomes following certain interventions.
