Again, the patient is supine with the knee flexed 60 to 90 degrees and the foot secured on the table. In an intact knee, voluntary quadriceps activation will result in slight posterior tibial translation. In PCL injury, however, with resisted knee extension and quadriceps contraction, the posteriorly subluxated tibia reduces anteriorly. This test may be useful in assessing relative anterior and posterior instability in patients with combined ACL and PCL deficiency.2 (Fig. 53-8).
