A comprehensive rehabilitation of the lower extremities is prescribed following this midfoot injury. Ambulation in a partial weight-bearing environment such as a pool may be an invaluable rehabilitation environment. Furthermore, the use of a bone stimulator may be a therapeutic adjunct to facilitate osseous healing.

Assessment and correction of biomechnical abnormalities are vital components of the rehabilitation program. A thorough lower extremity biomechanical examination is performed to assess for possible angular and rotational misalignment. The forefoot-hindfoot relationship is particularly important. Mechanical causes, such as improper shoe wear or training technique should be corrected (Fig. 70-15). Therefore, a custom foot orthosis may facilitate timely return to sport and prevent recurrences by accommodating for abnormal foot-ground relationships. Furthermore, athletes at risk of nutritional or endocrinologic causes of bone fragility should be thoroughly investigated.
