Thumb Boutonniere

Traumatic boutonniere injuries to the thumb are rare but have been reported. These injuries occur due to trauma to the thumb with hyperflexion of the MP joint. The patient presents with an extensor lag at the MP joint with or without interphalangeal joint hyperextension.

Pathology involves rupture or attenuation of the insertion of the extensor pollicis brevis and stretching of the dorsal radial capsule. Treatment may include splinting with the MP joint extended. Cardon et al83 has reported excellent results with surgical repair in which the extensor pollicis brevis is reattached to its insertion site and the dorsal radial capsule is imbricated.
