Visualization is critical to simplifying shoulder arthroscopy. Several maneuvers discussed previously help facilitate good visualization. These include positioning the patient, portal placement, and the use of the outside-in technique, interscalene block, use of a pump to control the pressure, controlled hypotension by the anesthesiologist, and the use of cannulas to control tissue swelling. If these maneuvers fail, other techniques may be implemented. For example, the fluid temperature may be kept lower to prevent bleeding.3 Additionally, epinephrine may be added to selected bags of fluid. A typical concentration is 1 mg epinephrine in one 3-L bag of saline (0.33 mg/L).6
